Our Assisted Living Program not only provides all of the Adult Home Care services but also includes: Nursing supervision, Nursing care, 24 hour Certified Home Health Aids, Personal Care aides, Physical , Occupational. And Speech Therapy.

Common Questions
How do I know which program I will qualify for?

A PRI and screen determines the level or care. This can be done through a visiting nurse service by obtaining a physician's order for PRI and screen trough your family physician. You may contact your local visiting nurse associating or return the physicians order to out facility and we will have out visiting nurse complete the PRI and Screen.

Is a medical evaluation required for the Adult Home?

Yes. We are required to obtain a competed Medical Evaluation from and if any past psychiatric history, a mental health evaluation must be completed.
