Needs
Target Population/Special Needs The Long Island
Center is designed to serve primarily the frail elderly. In
general, this population is in the over 65 category. Our Assisted
Living program is aimed primarily at those individuals who
are on SSI and are eligible for Medicaid.
Currently, the implementation or RUGs reimbursement
system appropriately encouraged skilled nursing facilities
to admit and care for the sickest and most intensive care
need residents. This has created a significant service gap
for many individuals who are now beyond retention standards
of adult care facilities but are not yet in need of the most
sophisticated levels of nursing care.
Admissions
An Admissions Director is available to provide
information to prospective residents and their families. Our
hours of admissions are from 9:00 AM to 5:00 PM. We request,
when possible, that a family member be present during the
admission to assist the resident in adjusting to their new
environment. At this time, all financial information, resident
responsibly, and general information is discussed.
Persons Eligible for assisted Living Program include those
people who:
a. Are medically eligible for placement in a
nursing facility but can be appropriately cared for in an
assisted living program (ALP). These people would require
placement in a nursing facility if the ALP was not available.
Such individuals could include people who no longer have a
home or for whom the home environment is not suitable place
in which to live and receive care services safely.
b. Are categorized by the long term care patient
classification system (Resource Utilization Groups) (RUG)
categories particularly those categories that formerly would
have been considered appropriate for placement in health related
facility (HRF). The Long Island Living Center Assisted Living
Program will admit persons who will meet the criteria for
the ALP program as set forth by the Department Of Health.
Medical Services
A complete medical Program includes: Dentistry
(offsite), Optical. Audiology, Podiatry, laboratory/x-ray,
Pharmacy and rehabilitation is available. All residents will
be assigned to a Physician unless they have their own. Residents
will be scheduled to see the physician every 60 days unless
the resident's needs warrant an earlier visit. All residents
must have a complete physical at least once a year.
Visiting Nurse Services
Registered Nurse's Aides are here to provide
each resident with care he/she requires. The cost of such
service is paid for by the resident's coverage and third party
billing.
